By Fax: Send your completed form to 301/495-0810. Fax registrations must include MasterCard, American Express or VISA payment.

By Phone: Phone your complete information to 301/495-0900. Phone registrations must include MasterCard, AMEX or VISA payment.

By E-mail: Send all registration information with credit card information to meetings@asis.org.

By Mail: Submit one copy of the registration form for each registrant and provide all information requested on the form. Send the completed forms to:

Early Registration Discounts
To qualify for registration discounts, your registration and payment must be received by ASIS by September 17, 1999. Late rates apply to registrations received after this date.

Members of the Special Libraries Association, Medical Libraries Association and American Association of Law Libraries may also register at the ASIS member rates. Include the name of your association and your membership number on the registration form.

Great News! - Special Discount
If this is your First ASIS Conference OR if you've never taken an ASIS Continuing Education course, you can SAVE BIG! Just register for the full conference AND any full day CE course and take $75 off your total registration fee. This offer is not available at the student or retired registration rate.

Annual Banquet and Proceedings
One admission ticket to the Awards Banquet, Wednesday, November 3 , and a copy of the Proceedings are included with each full conference registration except for student categories. Additional banquet tickets or copies of the Proceedings may be purchased on site.

Cancellations & Refunds
Cancellations and refund requests will be honored only if received in writing by October 1, 1999. No cancellation or refund requests will be accepted after this date. All refunds will be subject to a $25 processing fee. Refund checks will be issued six to eight weeks after the conference. Special event fees are non-refundable.
